Registration With The Practice

PreRegistrationWe would ask that any applicant to register with the Surgery please visit us in person with your Photographic Identification an Proof-of-Address as described in the "Registration" tab. This is so that you care needs can be dealt with in person by our staff members in one simple visit to the Surgery.

Registering online during Covid-19

We are accepting our Registration Forms (see below) via email whilst the Practice is closed to foot-traffic.

If an applicant 16 years of age and over downloads and completes their own forms (parents can complete the Children's form and GMS1 for those under 16) the documents can be emailed to the address where they will be processed in up to ten working days.

Normally we would ask for Photo-ID and a Proof-of-Address upon registering. If an applicant is able to photograph or scan their ID into a picture file (like a .jPeg) then we will use that ID. 

If you are unable to supply ID we will just accept the forms at this time. We will revert to pre-Covid processes when the virus situation has changed sufficiently.The Practice Boundary rules are still in effect.
