New Patient Registration
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
Please complete a GMS1 form for each individual applicant for registration. Please also complete a Patient Registration Questionnaire if you are 16 years old or over. If you are registering a child please complete the Children's Registration Form instead.
All Borough Birth Registrations have been closed which means new parents are unable to obtain a birth certificate.
Newborn babies can be registered at a GP Practice without a birth certificate.
A newborn baby can be registered with the details provided in the Red Book issued by the Midwife team at hospital.
Registering online during Covid-19
We are accepting our Registration Forms (see below) via email whilst the Practice is closed to foot-traffic.
If an applicant 16 years of age and over downloads and completes their own forms (parents can complete the Children's form and GMS1 for those under 16) the documents can be emailed to the address BHCCG.PavilionSurgery@nhs.net where they will be processed in up to ten working days.
Normally we would ask for Photo-ID and a Proof-of-Address upon registering. If an applicant is able to photograph or scan their ID into a picture file (like a .jPeg) then we will use that ID.
If you are unable to supply ID we will just accept the forms at this time. We will revert to pre-Covid processes when the virus situation has changed sufficiently.The Practice Boundary rules are still in effect:
We are no longer taking on new patients from outside our catchment area.
Patients who move out of the practice area will be asked to register with another GP nearer to their address.
In accordance with local guidelines all patients must provide two proofs of their identity when registering, e.g. birth certificate, marriage certificate, driving licence, passport, wage slip or NHS card. We will ask you to complete a registration form and a questionnaire to collect important medical data. Both forms can be downloaded below.

Some new patients will be asked to undertake a health check with a health care assistant.
Medical treatment is available from the date of registration. Please contact reception for further information.
Change Personal Details
If you change your name, address or telephone number please let our receptionists know or inform us of the change via emis access online.
If you move outside the practice area it will be necessary for you to find a doctor in your new area.
