Changing your contact details
To update details like your name, address, phone numbers or email address:
- fill out a change of contact details form
- use the NHS online services.
- visit the surgery from 9am to 6pm with a form of ID
We can only update details in person to verify your identity and prevent dictation errors. We cannot update any details without a form of ID.
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your date of birth or name
You will need to show us proof that you have changed your name or your date of birth is incorrect. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
